Duties are varied and personalised to the Service User’s needs such as:
- Supporting Clients e.g., housekeeping, cooking, shopping, domestic duties, budgeting etc, and helping them to maintain all aspects of their domestic lives with emphasis on Infection control in light of Covid 19 pandemic and any other Public Health initiatives as per the need.
- Supporting clients in all aspects of personal care (Where required).
- Supporting clients with their medication administration including ordering and recording and managing their medication depending on level of needs.
- Facilitating and supporting with social activities to create opportunities for PWS to develop establish and maintain personal relationships and social networks, which encourage inclusion and greater participation and integration in their chosen communities. This can be achieved by visiting family/friends, walks, cinema/theatre, meals out, going for a drive (where required), reading etc.
- Improving the everyday skills of PWS to foster their independence by encouraging and reassuring by supporting them physically and emotionally.
- Monitoring progress and recording detailed notes on Person Centred Software (MCM) relating to the support offered to the PWS assessing requirement for additional support where necessary.
- Recording and reporting of all the incidents, accidents and concerns in a timely manner.
- Liaise with families, friends, advocates, external agencies and the management team to provide a consistent approach that meets the individual needs of PWS contribute to weekly and monthly Key Worker Reports.
- Ensure appropriate care and support is delivered to meet individual needs, respecting personal dignity, culture and beliefs. Facilitate smooth teamwork throughout the day by helping service users with appointments and daily activities based on their assessments and care plans.
- Maintain a safe environment by preparing and cleaning rooms and equipment, focusing on infection control to ensure everyone’s safety, including service users, families, staff, and visitors and reporting issues to maintenance team on urgent basis.
- Complete detailed daily records on the Person Centred Software System (MCM) and completion of administrative tasks to enable effective and efficient client support.
- To support PWS with annual health checks and to access any additional health care provision enabling them to make an informed choice regarding any prescribed treatment, seeking advice where necessary.
- Continually update the client documentation including support plans, risk assessments, positive behavioural support plans as required.
