Support Worker

Team Leader

A Team Leader will provide direction, instruction and guidance ensuring the smooth running of our supported houses with our service user always at the centre. The Team Leader will know the individuals strengths and weaknesses and how to motivate colleagues to attain high levels of performance. The Team Leader will achieve co-operation, encourage progress, results and be a good listener. They will be the key point of contact with families and health professionals.

Senior Support Worker

The senior support worker will be the key member of staff for a particular service user, they will be responsible for implementing the care plans and ensuring relevance and accuracy. Good communication skills are vital and a desire to improve and develop our service users lives.

Manager

The Manager will undertake clinical audit and healthcare service monitoring and review as required by the commissioner and CQC.

Improve health and emotional well-being addressing health inequalities, ensuring access to health services, health promotion and appropriate care and support to manage long-term conditions.

Provide excellent leadership, direction and effective management of the team’s resources, and finances ensuring professional standards and best practice act as key drivers within a culture of continuous service improvement, value for money and safer practice.

Ensure continued focus on enablement and developing independence through self-directed support and achieving excellence in customer care and service delivery.
